Below is the link to the Parent Onboarding Packet for the East Tampa Youth Leadership & Entrepreneurship Academy.

This packet needs to be completed and received by the Academy staff to qualify your student as a member of the Academy for the current school year.
Please be sure you see the confirmation page immediately following your submission to be assured that your packet was received.
Parent Onboarding Packet Instructions
You will need to have access to the following information to complete the packet:

Student's email address
-
Student's phone number
-
Names of Student's medications, if any
-
Names and phone numbers for 2 Emergency Contacts

Medical Insurance information

PLEASE NOTE: The student WILL NOT qualify as a member of the Academy until ALL of the requested information is submitted.
If you have more than one student joining the Academy, a separate Parent Onboarding Packet must be submitted for each student.
